Themis Incident Report - PLEASE NOTE: PAPER FORMS WILL STILL BE USED WITHIN THE SCHOOL OF CHEMISTRY FOR THE TIME BEING. SEE NEW S3 FORM BELOW. For reporting incidents involving the environment or safety or "near misses". Now done on Themis. To report an incident, log in to Themis and select Create New Incident Report from Staff Self Service menu. For help with Themis incident reporting, click here.

*On 11/8/05 the Science Faculty EHS Committee agreed that due to work areas generally remaining static over the past 3 years, the quarterly inspections can be eased to half yearly inspections, unless there is a significant change in a particular work area.
